Patients rights and responsibilities

Health and Wellness Services (HWS respects, supports, protects all patient’s rights, recognizing that each patient is an individual with unique needs. We strive to provide considerate, respectful care for everyone. We do not limit ,deny or restrict healthcare on the basis of race, color, religion, ancestry or national origin, age, sexual orientation, gender identity and expression, physical or mental disability, citizenship status or any other basis protected by law.

As a patient, you have the right to:

Understand your rights and responsibilities.
Patients- and family-centered care that is safe, high-quality and considerate.
Clear information about your health status, treatment and care, benefits, risks and care-givers.
Take part in decisions about your treatment, care and services.
Have help making an advance directive and have your wishes as expressed in your advance directive honored.
Confidentiality and privacy in all matters.
Secure and safe an environment that offers dignity, including freedom from neglect or mistreatment.
immediate answers to concerns, complaints and requests for help, without fear of retaliation.
You will receive information in a language you understand.
Patient Responsibilities

As a patient, you have a responsibility to:

Accurately inform our team with your medical history symptoms,medications and provide any changes in your condition. Let us know if you have allergic reactions to medications.
Feel free to ask questions about your treatment and care, make sure information is clear.
Have an active role in your care planning.
Please make staff aware if you do or do not want anyone involved in your care or decision-making (family or friends).
Advise staff if you have an advance directive.
Ask your healthcare provider about managing pain and pain relief.
Respect all by rules and policies.
Try not to bring valuables to office visits.
No smoking on property including electronic nicotine, tobacco,or vapor products.
Please provide us with your insurance information.
Please make staff aware you cannot keep a scheduled appointment.
